Personal data transfer agreement
When filling out the form by any interested person (hereinafter referred to as the “User”) on the website https://isapsfast.ru (hereinafter referred to as the “Site”), the User provides ANO DPO UMC “InMedTech” with personal information, which is personal data in accordance with the law of the Russian Federation, including e-mail address, last name, first name, patronymic and other information necessary for registration on the Site. If the User refuses to provide ANO DPO UMC “InMedTech” with the specified information registration on the Site is not performed.
Hereby, in accordance with the Federal Law of July 27, 2006 No. 152-ФЗ “On Personal Data” (hereinafter – “ФЗ No. 152”), the User gives and confirms his consent to ANO DPO UMC “InMedTech” to process his personal data. This consent is valid indefinitely and can be revoked upon the written application of the User.

In accordance with clause 1, clause 3 of art. 3 of the Federal Law of July 27, 2006 No. 152-ФЗ “On Personal Data”, personal data is any information relating directly or indirectly to a specific or identifiable individual (subject of personal data). Processing of personal data – any action (operation) or a set of actions (operations) performed using automation tools or without using such tools with personal data, including collection, recording, systematization, accumulation, storage, clarification (update, change), extraction, use, transfer (distribution, provision, access), depersonalization, blocking, deletion, destruction of personal data. In accordance with clause 1, part 1 of Art. 6 of the Federal Law No. 152, the processing of personal data is carried out with the consent of the subject of personal data to the processing of his personal data.
